What to Bring to Your Appointment
Items patients are to bring to their appointment include the following:
- Insurance card
- Referral from primary care physician, if required by your insurance
- List of prescription meds
- Identification
- Completed patient forms
- Co-payment
- Pertinent labs and x-ray studies
- Pertinent outside medical records
If you are having a procedure done, the pre and post-operative instructions will be given to you by a nurse at the time of the initial pre-operative visit.

Payment and Insurance Information
Urology Associates of Houston, P.A., is a provider for Medicare and most major insurance plans. However, it is in your best interest to check your insurance providers' list of participating physicians to be sure your plan allows you to be seen at our office. Also, please review with your insurance provider the rules and limitations of coverage for your particular plan. Coverage decisions are between you and your insurance provider, although we do provide insurance billing.
